Meet our Home Care Package Case Manager

Our experienced and qualified Case Manager, Melanie Kelly can help you choose your Home Care Package services.  

What is a Home Care Package?

A Home Care Package provides you with Government funded services that will help you to remain at home for as long as possible.  They offer the choice and flexibility of care that’s needed by frail, older people to remain living independently at home.

How can I receive a Home Care Package?

 You will need to call My Aged Care on 1800 200 422 to arrange for an Aged Care Assessment (ACAS) to receive a Home Care Package. The ACAS assessment will work out what services you need at home and the package level that’s right for you.

When you receive a package

After your ACAS assessment, you will be placed in the National Queue until you are offered a package, which is when you can choose Cooinda as your provider. Your place on the National Queue will depend upon how long you have been waiting and with greater needs a higher priority.

You will need to choose a provider to Case Manage your Home Care Package, which is when you call Cooinda.  We will arrange for our Case Manager to visit you at a time that suits you to talk about your needs and help you decide what services you will have delivered into your home.

Cooinda Home Care Package Management

Our experienced Case Manager is a health care professional who will help you choose the support services right for you, so you can continue living independently in your own home.  They will also coordinate appointments and help you understand letters.

Already receive Home Care Package services from another provider?

If you already receive a Home Care Package, you can now change your provider to Cooinda.  Simply give us a call and talk to one of our Case Managers about making the swap.
